Medicaid Service Coordinator (Full Time)
Time Requirement

40 hours per week | Hours are flexible

Job Purpose

Provides coordination of clinical services and case management to consumers.  This includes development and implementation of all Individualized Service Plans, Behavioral Management Plans, and Incident Review.  Serves as liaison to all ancillary programs and coordinates referral and admission/discharge procedures.


  • Associate’s degree in health and human service field or RN.
  • One year experience working with individuals with developmental disabilities or service coordination with any population.
  • Valid NYS driver’s license.
  • CPR/First Aid as well as all other required training.

Direct Support Professional (Full Time, Part-Time, Per Diem) (Full Time)
Time Requirement

40 hours per week | Hours are flexible

Job Purpose

The Residence Counselor is responsible for providing on-going training in the acquisition of independent living skills and ensuring the health and safety of each individual. The position serves as a role model for others and enables the individual to strive for excellence while ensuring the quality of care.


  •  High School Diploma or equivalency. One year’s experience with the developmentally disabled is preferred.
  • Valid NYS drivers license and telephone.
  • Must have or complete within first 3 months and maintain SCIP-R, Medication Certification, CPR/First Aid as well as all other required training.
